Was up till almost 2 last night, tank is almost all plumbed in.
I noticed something - it is off level.
Never had to shim up a tank before - shim from floor to base of stand?

Definately want to shim the stand. Fish tanks always let you know what is wrong in your house!!

Slab could be off that much. Slabs are rarely sighted and the method for smoothing is pretty archaic. Slab also will slant towards the drain that is built in. It could be two shims over a 4-6 foot span quite easily.

I used to take a marble or a ball into basements and just let them roll around. Might slope more than one direction.
